Special group 'all'

A special group all is to be added that implicitly includes every user and guest (not logged in).

• all can be added to reviewer_groups. Then every user and guest has read access.
• all cannot be added to coauthor_groups to prevent erroneous write access. There is no use case.
• all is not a GroupObject but is handled as a special in the code.
